This practice has established safety measures to protect your privacy and confidentiality. Your confidential clinical notes will not be shared outside of the practice, without your written consent.


Everyone is entitled to confidentiality. The federal law, Health Insurance Portability and Accountability Act (HIPAA) strictly forbids disclosure of health information without the expressed consent of the patient. In the event you are a danger of causing physical harm to yourself or another, this practice will be obligated by law to inform the relevant authority to reduce and/or prevent such harm from taking place.

  • Patients are able to see and obtain copies of their medical records and request corrections if they find mistakes.

Cancelation Policy

We understand that unforeseen events in life can occur, and there are times you’ll need to reschedule your appointment. However, if you don’t show for your scheduled session or haven’t notified us at least 24- hours in advance, you will be required to pay a $200 late cancellation fee.
